Medical Surgical Registered Nurse Travel/Contract Position
Location: Dededo, Guam
Contract Length: 13 weeks
Job Details
- Specialty: Medical Surgical Registered Nurse
- Shift: Night (3x12 hours, 7 PM - 7 AM) or Day (3x12 hours, 7 AM - 7 PM)
- Hours: 36 hours per week (3 shifts of 12 hours)
- Weekend Requirement: Every other weekend; flexibility required to support facility needs
- Holidays: Working on holidays is required; specific dates determined by unit needs
- Shift Assignment: Travelers are scheduled primarily on one shift (day or night) without shift preference requests
- Return to Office (RTO): Limited to no more than 3 consecutive days and a maximum of 7 days total during the 13-week contract; no RTO approved for June
- Overtime: At management discretion, not guaranteed
- Locals: Candidates residing in Guam are not eligible
- Guaranteed Hours: Contracted hours guaranteed; client may cancel up to 3 shifts during the assignment
- Travel: Flights are not reimbursed; travelers should plan accordingly and schedule return flights after contract completion
- Additional: Travelers should be prepared to pick up additional shifts as needed due to census fluctuations

Required Qualifications
- Minimum of 1 year of Medical Surgical nursing experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Current BLS certification issued by the American Heart Association
- Current ACLS certification issued by the American Heart Association
Certifications (Must be current and digital certificates only)
- BLS (Basic Life Support): American Heart Association (AHA) provider card required; instructor cards not accepted
- ACLS (Advanced Cardiac Life Support): AHA provider card required; instructor cards not accepted; AHA RQI digital certificates accepted
- PALS (Pediatric Advanced Life Support): Preferred, AHA provider card required for certain units (ER, OR, PACU)
- NRP (Neonatal Resuscitation Program): Required for ER; must be fully completed prior to start (hands-on portion mandatory)
- Additional Unit-Specific Certifications: Conscious Sedation (ER, ICU, Cath Lab), ESI (Emergency Severity Index) for ER, NIH Stroke Scale Training, Pressure Injury Training, Alaris IV Pumps, Pyxis CareFusion (all required for RNs)
Compliance and Testing Requirements
- Background Check: 7-year criminal background check within 1 year of start; includes nationwide and international searches if applicable; no pending background checks allowed at start
- Drug Screening: 9-panel instant drug screen within 30 days of start; not required annually unless gap between assignments exceeds 30 days
- Health Screenings:
- Negative TB test or chest X-ray within 1 year prior to start
- Physical exam within 1 year (facility does not require but Aya recommends)
- Fit mask test within 1 year prior to start; must use approved masks or bring own PAPR if needed
- Immunizations:
- Measles (Rubeola), Mumps, Rubella: Proof of immunity by titer or vaccination; boosters required for negative/equivocal titers; declinations accepted only if born before 1957
- Varicella: Positive titer or 2 vaccines required; booster required for negative/equivocal titers
- Hepatitis B: Immune titer or full vaccine series required; declinations accepted only if born before 1957
- Flu Vaccine: Mandatory during flu season (Sept 1 - June 30); medical exemptions considered with facility-specific form
- COVID-19 Vaccine: Strongly preferred; declination requires facility-specific waiver form
Orientation and Onsite Testing
- Drug Calculations and Basic Knowledge Assessment Tool (BKAT):
- Administered onsite during orientation
- Pen and paper format, must be completed in person
- Two separate exams, 10 questions each, 80% passing score required
- Two attempts allowed with remediation discussion after first attempt
- Facility-Specific Modules:
- Nursing modules (8-12 hours) including Pressure Injury, NIH Stroke Scale, Alaris IV Pumps, Pyxis CareFusion, and others must be completed by compliance deadline
- Certificates of completion must be uploaded as required
Additional Notes
- Candidates should plan to arrive at least one day before the start date due to long flight times and time zone differences
- All certifications expiring within 6 months of start must be renewed prior to assignment start; some renewals can be completed onsite
- Facility does not accept printed or photographed copies of certifications; only digital certificates are valid
- Candidates must complete all required facility-specific trainings and upload certificates accordingly
References and Work History
- Two professional references required for travel, allied, and per diem acute care positions (within 3 years)
- Work history verification required for the last 10 years; attestations accepted if verification is not possible
This position requires a highly flexible, licensed Medical Surgical RN with current certifications and the ability to work rotating shifts, weekends, and holidays in an acute care trauma facility in Guam. Candidates must be prepared for onsite testing, compliance with strict health and background requirements, and travel logistics without reimbursement for flights.
