Registered Nurse - Home Health
Location: Omaha, Nebraska
Employment Type: Long Term Contract
Contract Length: 13 weeks
Shift: Monday to Friday, Days (AM shift)
Coverage Area: Omaha Metro and surrounding territories within a 60-mile radius
Job Description
- Provide home health nursing care with productivity expectations of 28-32 points per week based on a 40-hour workweek.
- Visit types include Start of Care (SOC), Recertification, Routine visits, Discharge (OASIS), and Resumption of Care (ROC).
- May be asked to assist in other territories within the service area as needed.
- Overtime requires prior written approval from the Administrator.
- Two weeks cancellation notice required.
Required Qualifications
- Valid RN license in Nebraska
- Minimum of 1 year of home health nursing experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Experience working in a home health setting
- Proficiency with HomeCare HomeBase (HCHB) software

- SOC: 2.5 points
- ROC: 2.0 points
- Discharge (OASIS): 1.5 points
- Recertification: 1.5 points
- Routine visits: 1 point
